This six week workshop teaches practical skills for living a healthy life with an ongoing condition such as asthma, diabetes, high blood pressure, kidney/renal failure, heart disease, and arthritis. Developed by Stanford University’s Patient Education Department, it is designed to help people learn to manage their Chronic Conditions and improve the quality of their lives. It is free to patients
