Take charge of your health on National Healthcare Decisions Day
National Healthcare Decisions Day, which takes place Tuesday, April 16, is an initiative to encourage patients to express their wishes regarding health care and for providers and facilities to respect those wishes, whatever they may be.
This event was started six years ago to inspire, educate and empower the public and providers to think about — and talk about — advance care planning.
At MultiCare, we are providing a number of opportunities for providers, staff members, patients and their family members to incorporate several tools into these important discussions:
- 5 Wishes – A specific Advance Directive document that helps patients and family members document their wishes in detail
- The Conversation Project (TCP) Starter Kit – This helpful tool facilitates families’ abilities to engage in discussions regarding a variety of scenarios that commonly occur as one’s health declines
- How to Talk to Your Doctor – This document provides ideas to improve communication to make sure patients, family members and those on the health care team are “on the same page” when it comes to understanding a patient’s medical conditions, as well as exploring the best means to align patients’ goals and values with medical interventions.
The documents listed above will be available at MultiCare hospitals and clinics, and may be scanned into patients’ charts for easy access in the future. We are also encouraging our providers and staff to address their personal advance care planning.
Remember, the time to make plans for future health care decisions is now, so you can ensure your voice will be heard, and your wishes carried out, no matter what your future might bring.
