Living Well with Chronic Conditions

This six week workshop was developed by Stanford University’s Patient Education Department. It is designed to help people learn to manage their Chronic Conditions and improve the quality of their lives. It is free to patients. There is an optional book that compliments the course that can be purchased for $20.

  • Working with your Healthcare team
  • Relaxing and managing stress

  • Solving problems effectively
