Living Well with Chronic Conditions Workshop

This six week workshop teaches practical skills for living a healthy life with an ongoing condition such as asthma, diabetes, high blood pressure, heart disease, and arthritis. Developed by Stanford University’s Patient Education Department, it is designed to help people learn to manage their Chronic Conditions and improve the quality of their lives. It is free to patients. There is an optional book that compliments the course that can be purchased for $20.

  • Eating Well
  • Designing your own exercise program
  • Managing medications
  • Working with your health care team
  • Relaxing and managing stress
  • Setting goals
  • Solving problems effectively
  • Dealing with emotional changes
