Diabetes
HA1c (Hemoglobin A1C) measures blood sugar control in diabetes. Blood test for HA1C level at least once in the last year for diabetics age 18 - 75.
Having better blood sugar control in diabetes (indicated by a lower A1c level which means a lower average blood sugar level), helps to significantly reduce diabetic complications (such as eye and nerve damage, kidney failure, heart attacks and strokes).
Blood sugar control in the last year (<8% Ha1c).
NCQA Diabetes Recognition Program Target is 60%
LDL-C testing at least once in last year for Type I or Type II diabetics age 18 - 75, at least one primary care visit in last year.

Last LDL-C < 100 (in last year) for Type I or Type II diabetics age 18 - 75, at least one primary care visit in last year.
NCQA Diabetes Recognition Program Target is 36%
The percentage of patients 18 - 75 diagnosed with diabetes (Type I or Type II) who had an eye exam at least once in a two-year period, or it there is evidence of eye disease, during a one-year period.

Last BP < 140/90 (in last year) for Type I or Type II diabetics age 18 - 75, at least one primary care visit in last year.
High Blood Pressure
Blood Pressure Control in Hypertension
Last BP in year for patients diagnosed with hypertension less than 140/90, age 18 to 85.
