background screenings
MultiCare conducts a background check including a search of Washington State Patrol records and a third party background investigation. Before we can initiate your background checks, we will need signed copies of four forms. See the link on the right side of this page.
Washington State Patrol: This enables us to do a search of Washington State Patrol Records. Please fill out Sections C & D. Fingerprints are not required at this time - (only signature is required.) Please return the form to MultiCare (do not mail it to WSP).
Consumer Report/Investigative Consumer Report/Disclosure and Release of Information Authorization: This is the second part of the background check and authorizes a third-party vendor to conduct a background investigation.
Disclosure Form: Please complete all the pages of the form, sign and return.
Academic/Education Verification: Please indicate your post high school academic/education information. For some positions, your education will be verified by a third-party vendor.
Once completed, these forms can be faxed or scanned and emailed back to MultiCare.
pre-employment / health evaluation meeting
Please bring the following items to your Pre-Employment / Health Evaluation meeting. These items verify your legal right to work for MultiCare, ensure that you will be paid in a timely manner and help us ensure a safe environment for you, your co-workers and our patients.
- I-9 documentation. The two most common forms of documentation are a social security card and driver’s license. A complete list of approved documents is included in the offer letter email. Original documents are required.
- Voided check (checking) or deposit slip (savings) for the bank account where your paychecks will be deposited.
- Original licenses, certifications or registrations if applicable.
- CPR Card (if required for your role): Please note that MultiCare requires the American Heart Association (AHA) Health Care Provider card.
- If you have immunization/shot records, please bring them with you.
- At the health evaluation, we will verify that you are immune to certain diseases and not carrying active Tuberculosis (TB). If necessary, you will be immunized for measles, rubella, chickenpox and hepatitis B.

At this Pre-Employment / Health Evaluation meeting, you will be given more information about New Employee Orientation (your first day of work). We will be taking a picture of you for your Employee ID Badge. You will receive the ID Badge at New Employee Orientation (your first day of work.)

- Parking is available at the Jackson Hall Parking Garage and will be validated for this Pre-Employment / Health Evaluation meeting only.
- Good Samaritan Hospital Location:
- Please see the Parking Map for Good Samaritan Campus for the Pre-Employment / Health Evaluation meeting.
Validation is not available for at any other location.
e-verify
MultiCare Health System participates in the United States Department of Homeland Security's E-Verify program.
This program enables MultiCare to use an online eligibility verification system for applications submitted. By using this program, we are able to be fully compliant with Federal law, which requires all employers to verify the identity and employment eligibility of all persons hired to work in the United States.
