Required Forms
Before you start
As a new hire, you must have a health evaluation before your first day of work. This will help us insure a safe environment for you, your co-workers and our patients. At the health evaluation, we will verify that you are immune to certain diseases and not carrying active Tuberculosis (TB). If necessary, you will be immunized for measles, rubella, chickenpox and hepatitis B.
Information regarding your Health Evaluation will be given to new hires prior to NEO by HR. If your NEO location is in Tacoma and you have questions or are able to set up an appointment before attending NEO, please contact Employee Health at 253-403-1007. If you have questions and your NEO location is in Puyallup, Employee Health can be reached at 253-697-1560.
Documentation
Depending on your role and location, some new hires will have completed documentation paperwork with HR before NEO. If you do not complete your new hire paperwork with HR prior to NEO, you will need to bring the following items with you to orientation. These items verify your legal right to work for MultiCare Health System and ensure that you will be paid in a timely manner.
- I-9 documentation: The two most common forms of documentation are a social security card and driver’s license. A complete list of approved documents is included in the new hire offer letter packet.
- Voided check: (checking) or deposit slip (savings) for the bank account that will receive your paychecks
- Original licenses, certifications or registrations (if applicable)
- Health Care Provider CPR Card (if required)
