Required Forms
Before You Start
As a new hire, you must initiate a health evaluation before your first day of work. NEO is your first day of work. Please contact Employee Health on the Tacoma General/Mary Bridge campus at 253-403-1286, or at the Good Samaritan Hospital campus 253-697-1560 to schedule your appointment. This will help us insure a safe environment for you, your co-workers and our patients. At the health evaluation, we will verify that you are immune to certain diseases and not carrying active Tuberculosis (TB). If necessary, you will be immunized for measles, rubella, chickenpox and hepatitis B. A skin test for TB will also be placed at your initial appointment. It will be examined a few days later. Follow-up TB tests will need to be scheduled yearly as part of your mandatory requirements.
