LabsNW will bill clients monthly with an itemized invoice that includes patient name, date of service, test performed, and the test fee. Additional information can be included if the client desires. These invoices are due upon receipt.
Laboratories Northwest will directly bill third party payors upon request. Complete the test requisition and include the patient's address and telephone number, date of birth, sex, subscriber’s name, address, and birthdate, physician's name, and diagnosis codes to their highest level of specificity. Also include the insurance company, insurance billing address, the policy number and member/group ID codes. If the required information is not provided, the patient may be billed directly.
We bill Medicare and Medicaid as well. Please provide information including the patient's Medicare or Medicaid number, address, age, sex, and diagnosis codes to their highest level of specificity. For Medicare patients, an Advance Beneficiary Notice may need to be completed by the client office before submitting collected specimens to the laboratory. Additional information regarding Medicare billing is explained under Medical Necessity on the next page.
Laboratories Northwest will bill private pay or non-insured clients directly. Please complete the billing information on the test requisition. Provide the patient's (or responsible party's) address and telephone number. Once testing is complete, patients may expect a bill 2-3 weeks after services
Western Washington Pathology, a private practice of Pathologists, provides pathology services for LabsNW. When pathology services are provided, two bills are issued: one from Western Washington Pathology (for physician services), and one from MultiCare (for the technical preparation of the specimens for pathologist review).
