Drug & Nicotine Screenings

MultiCare is a smoke-free, drug-free workplace. All employment offers are conditioned upon acceptable pre-employment drug tests which include testing for the use of marijuana and nicotine. As part of our commitment to a healthy workplace, we require employees to obtain an annual flu immunization.

Background Screenings

MultiCare conducts a background check including a search of Washington State Patrol records and a third party background investigation. Before we can initiate your background checks, we will need signed copies of four forms:

Washington State Patrol: This enables us to do a search of Washington State Patrol Records. Please fill out Sections C & D only. Signature required. Please return the form to MultiCare. Please do not mail it to WSP.

Consumer Report/Investigative Consumer Report/Disclosure and Release of Information Authorization: This is the second part of the background check and authorizes a third-party vendor to conduct a background investigation.

Disclosure Form: Please complete all three (3) pages of the form. Signature required.

Academic/Education Verification: Please indicate your post high school academic/education information. For some positions, your education will be verified by a third-party vendor. (If no post-high school academic/education, please note "Not Applicable" on the form.)

These forms can be downloaded using the link on the right side of this page. Once completed, all of these forms can be faxed or scanned and emailed back to MultiCare.
