How to Check Out Life Safety Books

To assure that the checking out of life safety books goes smoothly, please follow the guidelines and steps below.

When checking out books from the Wagner Library, the MultiCare employee is responsible for the timely return of the items in a unmarked and reasonably good condition.

The Life Safety Books can only be checked out within 45 days before the start of the life safety class being taken.

To check out a Life Safety Book a “Life Safety Consent” Form must be filled out with the requested book titles. The form is available at the Library and online on the Wagner Library web site under the “Life Safety Books” link. When completing the form, be sure to sign your name and fill in your badge number and your day time phone number. You can bring the form to the Wagner Library or if the work location is away from the MHS Campus, fax it to 253-403-1394. If your work location is off site, the book(s) will be mailed to you.

Life Safety Books can only be checked out at the Library by library staff between 6:30 AM and 6:30 PM Monday through Friday.

Life Safety Books are due back at the Wagner Library within 7 days of the conclusion of the Life Safety class. The recommendation is that the books are returned to library staff. Leaving it at the library when staff is not present, the employee will still be responsible for the loss if the book is not located.

If the checked out book is not returned or brought back in poor condition, the employee will be held responsible for the replacement of the book. Be sure not to forget the CD-ROMs, the DVDs, and cards inside many of the books.
