MultiCare gives you, or your authorized representatives, two options for accessing your MultiCare medical records:
MyChart is MultiCare's secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record, including:
If you don't already have a MyChart account, you can request an account access code from the MyChart home page.
If you would like to assign a family member or someone you have a close relationship with, access (proxy) to your MyChart account, you must complete the MyChart Proxy Access Release of Information Request Form, and send to the address listed below. You can also complete this form electronically at your next visit at MultiCare.

*Please Note: WA State law allows up to 15 business days to respond to a medical records request, but in most cases your request will be processed within 5 business days.
If you have additional questions, call the Health Information Department at 855-673-2673 (toll-free). We are open Monday through Friday; 8am to 4:30pm, and closed on holidays.
Imaging films/CDs (x-rays, MRI scans, CT scans) are processed through the Imaging Records Management Department. You will have to complete our release form to get a copy of these records sent to you.
