Access to Your Medical Records

Request Medical Record Copies - COVID-19 Test Results

There are two ways you can request copies of your COVID-19 Test Results:

  1. MyChart is MultiCare's secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record.
  2. If you are unable to access MyChart Complete a release form or send a signed and dated letter specifying what is to be released and to whom. We have included instructions on how to complete form with the release form. You can email your completed release form to our dedicated email address: [email protected]  We will then process your request and securely email your results to the email we received your request from or to the email address indicated on the release form. It may take up to one business day to process your request.

Use MyChart to Access Your Medical Records

MyChart is MultiCare's secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record, including:

  • Test and imaging results information
  • Allergy information
  • Past medications
  • Past visit information, including patient instructions

If you don't already have a MyChart account, you can request an account access code to get started.

Assigning a “Proxy” to Access Your MyChart Account

If you would like to assign a family member or someone you have a close relationship with, access (proxy) to your MyChart account, you must complete the MyChart Proxy Access Release of Information Request Form, and send to the address listed below. You can also complete this form electronically at your next visit to MultiCare.

Request Medical Record Copies - Medical /Surgical

There are two ways you can request copies of your medical records:

  1. Complete a release form or send a signed and dated letter specifying what is to be released and to whom. We have included instructions on how to complete form with the release form. You can either fax, mail, or scan and email the form to us as noted below:
    • Fax: 253-333-2419 (only monitored Monday-Friday; 8am-5pm) or;

    • Scan and email the completed form to [email protected]
  2. Submit an electronic request for records via our secure online portal.
    Medical Records Copy Fees for Patients
    *Please note: You may only request copies of your own record via the portal. If you are requesting records on another patient (including your minor children), then you will need to use option #1.
     
  3. You may contact the Health Information Department toll-free at 855-673-2673.

*Please Note: WA State law allows up to 15 business days to respond to a medical records request, but in most cases your request will be processed within 5 business days

Request Medical Record Copies - Outpatient Behavior / Substance Abuse

There are two ways you can request copies of your medical records:

  1. Complete a behavioral health release form and/or a substance use release form or send a signed and dated letter specifying what is to be released and to whom. You can either fax, mail, or scan and email the form to us as noted below:
    • Fax:253-697-8393 (only monitored Monday-Friday; 8am-4:30pm) or;

Requests for Imaging Records

Imaging films/CDs (X-rays, MRI scans, CT scans) are processed through the Imaging Records Management Department. You will have to complete our release form to get a copy of these records sent to you.

There is no cost for copies of your imaging records.
