There are two ways you can request copies of your COVID-19 Test Results:
MyChart is MultiCare's secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record, including:
If you don't already have a MyChart account, you can request an account access code to get started.
If you would like to assign a family member or someone you have a close relationship with, access (proxy) to your MyChart account, you must complete the MyChart Proxy Access Release of Information Request Form, and send to the address listed below. You can also complete this form electronically at your next visit to MultiCare.

*Please note: You may only request copies of your own record via the portal. If you are requesting records on another patient (including your minor children), then you will need to use option #2.

2. Complete a release form or send a signed and dated letter specifying what is to be released and to whom. We have included instructions on how to complete form with the release form. You can either fax, mail, or scan and email the form to us as noted below:

Imaging films/CDs (X-rays, MRI scans, CT scans) are processed through the Imaging Records Management Department. You will have to complete our release form to get a copy of these records sent to you.
