Access to Your Medical Records

MultiCare gives you, or your authorized representatives, two options for accessing your MultiCare medical records:

  • View your information through the appropriate Patient Portal account
  • Request a complete copy of your medical records

Instructions for Puget Sound-area Patients

The instructions below will walk Puget Sound-area patients through the differents steps to take to access your medical records. If you are a patient in the Inland Northwest-area, instructions for accessing your medical records can be found in the medical records pages for MultiCare Deaconess Hospital, MultiCare Valley Hospital and MultiCare Rockwood Clinic.

Use MyChart to Access Your Medical Records

MyChart is MultiCare's secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record, including:

  • Test and imaging results information
  • Allergy information
  • Past medications
  • Past visit information, including patient instructions

If you don't already have a MyChart account, you can request an account access code from the MyChart home page.

Assigning a “Proxy” to Access Your MyChart Account

If you would like to assign a family member or someone you have a close relationship with, access (proxy) to your MyChart account, you must complete the MyChart Proxy Access Release of Information Request Form, and send to the address listed below. You can also complete this form electronically at your next visit at MultiCare.

Request Medical Record Copies - (Medical /Surgical)

There are two ways you can request copies of your medical records:

  1. Complete a release form or send a signed and dated letter specifying what is to be released and to whom. You can either fax, mail, or scan and email the form to us as noted below:
    • Fax:

    • Mail:

    • Scan and email the completed form to [email protected]
  2. Submit an electronic request for records via our secure online portal.
    Medical Records Copy Fees for Patients
    *Please note: You may only request copies of your own record via the portal. If you are requesting records on another patient (including your minor children), then you will need to use option #1.

*Please Note: WA State law allows up to 15 business days to respond to a medical records request, but in most cases your request will be processed within 5 business days

Request Medical Record Copies - Outpatient Behavior / Substance Abuse

There are two ways you can request copies of your medical records:

  1. Complete a release form or send a signed and dated letter specifying what is to be released and to whom. You can either fax, mail, or scan and email the form to us as noted below:
    • Fax:253-697-8393 (only monitored Monday-Friday; 8am-4:30pm) or;
    • Mail:

If you have additional questions, call the Health Information Department at 855-673-2673 (toll-free). We are open Monday through Friday; 8am to 4:30pm, and closed on holidays.

Requests for Imaging Records

Imaging films/CDs (x-rays, MRI scans, CT scans) are processed through the Imaging Records Management Department. You will have to complete our release form to get a copy of these records sent to you.

There is no cost for copies of your imaging records.
