Anticoagulation Center

Monitoring & Education for Patients on Anticoagulant Therapy

MultiCare Rockwood's Anticoagulation Center, established in 1999, provides consistent monitoring and education for patients on anticoagulant therapy. We focus on prventive care to prevent conditions such as deep vein thrombosis, pulmonary embolism, stroke or other thromboembolic injuries associated with anticoagulant therapy.


  • Intervention to minimize or avoid complications associated with anticoagulant therapy
  • Education about how to reach therapeutic goals
  • Regular monitoring of international normalized ratio (INR) with dose adjustments as needed
  • Pre- and post-operative instructions for surgical procedures
  • Monitoring of other specialty procedures requiring the interruption of anticoagulant therapy
  • Oral or injection therapies using Warfarin or low molecular weight heparins
