Diabetes management

Medication Treatment for Type 1 or Type 2 diabetes

MultiCare Health System’sPharmaceutical Resource Clinics offer diabetes medication management for people diagnosed with Type 1 or Type 2 diabetes. At each visit , you will receive education and instruction by a pharmacist experienced in diabetes treatment.

Clinic introduction

Your physician will need to complete a referral for you to be seen for diabetes medication management. You will be contacted by the clinic staff to schedule an initial appointment.

Services provided

A pharmacist will work with you to design a medication regimen to improve your blood sugar control and meet A1c goals. You will meet with a pharmacist trained in diabetes management for an initial diabetes medication consultation and follow-up as needed.

The pharmacist can start, continue, change or stop diabetes medication, as well as order necessary labs and testing supplies. The pharmacist cannot replace your doctor for diabetes checkups and other standards of care. Our pharmacists work in partnership with your doctor to help you achieve your A1c goals. Once your blood sugar goals are reached, then diabetes medication management will be turned back over to the referring provider.

Contract of care

If you are unable to keep your appointment, please notify the office to reschedule. Failure to keep your appointments could result in termination from the program. Please notify the clinic if your telephone number changes.
